Filing a Complaint

PROCESS MEDICATION ERROR REPORT

If you believe an error has been made in the prescribing of a medication, the Board of Pharmacy has a process for investigating your claim. The Board will start the review process by collecting some background information. You will need to supply the following information:
    Date of error
    Patient's name
    Name of person filing the complaint
    Address to mail a complaint form to
    Phone number of the person filing the complaint
    Pharmacy address that made the error
    Description of the error and additional comments you feel might help in the investigation.
Direct your seven background questions to Jan Atkinson, Senior Compliance Officer by e-mail or phone (208) 334-2356. Please be detailed and thorough with your information so that the Compliance Officer can review the error and investigate further if needed.
